*Each participant must complete a release form in order to enroll. Each family team must complete one registration form and pay $10.00 per family.
*Registration is Saturday, Feburary 2, 2008 from 10am - 12 noon at the Health First Center at teh Valley Hills Mall. Final deadline for registration form, payment and release form is Staurday Feburary 9th.
